  • Hep B- given at birth or 1st doctor visit

  • 2 months-Hep B+Dtap+IPV, PCV, Hib & Rotavirus

  • 4 months- Dtap+IPV+Hib, PCV & Rotavirus

  • 6 months- Hep B+Dtap+IPV, PCV, Hib & Rotavirus

Frequently Asked Questions

  • There is a $50 no-show fee for all missed appointments. We reserve the right dismiss patients from our practice who fail to keep their scheduled appointments.

  • If you are more than 15 minutes late, we will try to work you in. In the unlikely event we are unable to, you may be asked to reschedule.

  • For non-controlled substances, please call your pharmacy and request a refill authorization to be sent to our office.

    For controlled substances, you may submit a request online, call our office at 817-861-2288, or email us at nussbaumoffice@yahoo.com. Please be sure to include the child's name, DOB, and medication requested. Please allow 24 hours before picking up a prescription. We are not able to call in or fax controlled substances, we apologize for any inconvenience. We can mail prescriptions upon request

  • If it is after office hours, you may call our office, which will then direct you to the answering service. The answering service will page Dr. Nussbaum and she will contact you shortly. If it is decided that your child needs prompt medical attention, she will refer you to one of the following:

  • Yes, we do our very best to accommodate same day sick visits. We understand the urgency of being seen the same day. We do not accept walk-ins. Please call our office to schedule a sick visit

  • Yes, for daycare and camp forms, you must have had a well-child visit within the 12 months. For infants, you must be current on your most recent well exam (ex. 2mo, 4mo, 6mo) Please allow 24 hours for those forms to be completed.

    There is a $45 charge for all sports physicals. Sports physicals require a doctor's visit. Sports physicals are not good exams. During your office visit, if cleared, you will receive the form the same day.
